Contract Employee Benefits
Granite Solutions Groupe offers medical and dental plans to all eligible contract employees. An employee becomes eligible after a 30 day waiting period from the date of hire. Effective date for coverage to begin is the first day of the month after 30 days of employment. Granite Solutions Groupe contributes the employee premium with the option for family coverage at employee expense.

to have the plan summary mailed to you by e-mailing email@example.com.
CalChoice Application and Principal Dental Application
– All eligible employees will need to complete the application form. The
form may be faxed to expedite the process but the original must still be mailed
to Granite Solutions Groupe’s corporate office.
Should you elect to participate in our health plan(s), your portion of the
premium(s) will be deducted from your payroll pre-tax, semi-monthly.
Group Life Insurance
The Company offers contract employees who have been employed by
Granite Solutions Groupe for 30 days a 100% employer-paid basic
group term life policy along with an accidental death and dismemberment
policy. Each policy generally pays a death benefit and Accidental
Death & Dismemberment amounts up to $35,000. There is no requirement
to submit to a medical physical or fill out a medical history questionnaire
for the $35,000.00 benefit. You may also elect to apply for an increase
in coverage up to $50,000.00.
The waiting period for all new eligible employees is the first
of the month following 30 days of full time employment
with Granite Solutions Groupe.
The Enrollment and Evidence of Insurability forms are provided below. Completed
forms can be faxed to Granite Solutions Groupe, but the original form(s) must
still be mailed to our corporate office.
Evidence of Insurability Form - If you elect the $35,000 coverage complete page 4 only "Enrollment Form"
and mail to Granite Solutions Groupe. If you elect to apply to increase your coverage, complete all 4 pages,
"Evidence of Insurability" and the "Enrollment Form" and mail to Granite Solutions Groupe.

Granite Solutions Groupe offers a voluntary pre-tax 401(k) plan to all eligible
employees. There are a variety of funds for you to choose from.
The Enrollment Kit includes an Enrollment form, Beneficiary form and Rollover form. To view detail information on the available funds, such as the prospectus, you will need to go online to www.mykplan.com. This information should be available within seven to ten days after your date-of-hire. You will enter your social security number then click on “Forgot your User ID” and you will be guided through the setup process. You will have the option to view the prospectus online or request copies to be mailed to you.
If you are interested in participating in the 401(k) Plan, you will need to
enroll via the Web or Voice-Response System as well as complete the Beneficiary form. Please mail the completed forms to:

CONTRIBUTIONS & VESTING SCHEDULE
The IRS establishes a maximum dollar amount that an employee can set aside each year from their salary before taxes are withheld to invest for retirement. Granite Solutions Groupe has established a minimum of 2% and a maximum of 50% of compensation that a participant may elect to defer. Once you enroll in the plan, the percentage you specify from each paycheck will be withheld and the money will be invested as you direct. Employer matching contributions are at the sole discretion of Granite Solutions Groupe.
Employee is 100% fully vested after 3 years of continuous service. The employment period is the 12 months beginning on the date of hire. You are always 100% vested in your own contributions, and any rollover contributions (if applicable) as adjusted for any earnings or losses on these contributions.

Employee Payroll / Reimbursement
Granite Solutions Groupe offers contract employees direct deposit for payroll.
Employees are paid on a semi-monthly basis, the 15th and the last day of the month;
provided the employee’s timesheet has client authorization and is submitted
per the time sheet policy.
- The authorization form will need to be completed and faxed or mailed to our
corporate office. Your request for direct deposit will be coordinated with our
payroll system and processed as soon as feasible.
GSG Expense Report Template
- Employees who incur authorized expenses by the client will submit receipts and
a client signed expense statement to Granite Solutions Groupe for reimbursement.
Granite Solutions Groupe's policy is to reimburse within 5 days of receipt of funds from GSG's client.
Note: Direct deposit is for payroll only; expense reimbursement checks
will be mailed.
